About the role | He kōrero whakamārama mō tēnei tūranga

We have an exciting opportunity for an Intensive Reintegration Facilitator to join the New Plymouth team on a fixed term full-time basis.
08:30 - 17:00 five days per week. Days set as per three-month rotating roster. Weekend work required.

Our Supported accommodation services is a partnership with the Department of Corrections and involves working with clients to plan their reintegration into the community following release from prison.

You will support people to reintegrate successfully into the community as valued citizens by identifying their needs (e.g. employment, education, housing, health, life skills, social relationships) and supporting them to access the services required to meet those needs.

Responsibilities include but are not limited to:

  • Develop appropriate plans in relation to client assessment, goals and outcomes
  • Be responsible for ensuring client referrals, assessments goal planning, risk identification and relationship management are followed and accurately documented in a timely manner
  • Work in partnership with external services/agencies
  • Engage and connect with the client, their family and their community

What you will bring | Ngā tohungatanga mō tēnei tūranga

  • An empathetic nature with a natural ability to connect with and build positive relationships with a range of people from diverse backgrounds
  • Experience in a relevant field (e.g. Mental Health, Addictions, Corrections)
  • Awareness of and respect for professional boundaries
  • High emotional intelligence and resilience
  • Good level of literacy (due to documentation requirements)
  • Level 4 or higher qualification in a relevant field or a willingness to work towards this
  • A full driver's license.
