He kōrero whakamārama mō tēnei tūranga | About the role
Care coordination is a key function within Access, the ever-changing and unique needs of our clients need and our support worker availability mean the linking of clients and support workers is subject to continual change. The role of the Regional Care Coordinator Team Leader ensures client support needs are met by trained and available workers, maintaining client satisfaction and Access's reputation. Your ability to think quickly, make decisions and work in a fast-paced environment will ensure your success in this key role.
Takohanga | Responsibilities
- Working as part of the regional leadership team to deliver quality home and
community support services to our clients
- Facilitating co-operative and cohesive working relationships between our
Nursing, Care Coordination and Support Worker teams
- Leading a team of care coordinators to facilitate the delivery of a responsive
client focussed home support service
- Ensuring support workers are rostered safely and effectively to meet the needs
of our clients
- Overseeing all new referrals to ensure effective rosters are put in place
- Managing escalations and complaints
- Managing team appraisals and feedback
Salary and Shifts

Salary: $29 per hour

Shifts: Monday to Friday, 8:00 AM - 5:00 PM

Hours: 40 hours per week

- Experience in leading a busy team preferably in a Healthcare environment
- Excellent interpersonal skills
- Experience in planning and prioritisation
- Excellent written and oral communication skills

- Strong team player who is able to work effectively with a wide range of people
- Excellent computer skills.
