Contract: Permanent, Part Time 24 hours/week (0.6 FTE)

About the Role

  • Provide professional (discipline specific) services as a member of the mental health Multi-Disciplinary Team (MDT).
  • Provide professional discipline specific services and care management for tangata whaiora/clients, their families whānau and caregivers.
  • Liaise and consult with tangata whaiora, consumers, their families, whānau and caregivers.
  • Support people with mental health problems to access appropriate supports and resources in the community.
  • Provide clinical mental health services in accordance with policies.

This service coordinator position within MHNASC includes:

Manor Park - Kelvin House Responsibilities:

  • Review and screen referrals received from regional NASCs.
  • Facilitate meetings with referrers and the prioritisation panel to support decision-making.
  • Conduct quarterly reviews of all individuals on the facility - s waitlist.

Broader MHNASC Responsibilities:

  • Deliver comprehensive assessments and collaboratively develop support plans with the individual, their whānau, clinical providers, and Non-Government Organisation (NGO) services.
  • Support individuals and their whānau in achieving optimal health, well-being, and safety through tailored assessment, education, and planning.
  • Actively participate in professional development activities to maintain and enhance nursing practice.
  • Role model best practice, including cultural responsiveness, within the multidisciplinary team environment.
  • Engage with community agencies to help achieve outcomes identified through the multidisciplinary team process.
  • Coordinate and facilitate services across districts and agencies to ensure seamless support.
  • Report operationally to the MHNASC team leader and maintain professional accountability to the relevant professional leader, MHAIDS.

The MHNASC office is located in Porirua. We assess individuals within Hutt Valley and Capital, Coast areas.

About you

  • Registered mental health professional with current practising certificate and a minimum of 3 years clinical experience.
  • Demonstrates an understanding of the significance of and obligations under Te Tiriti o Waitangi, including how to apply Te Tiriti principles in a meaningful way in the service coordinator role with a commitment to achieving equitable outcomes for Māori.
  • A personal commitment to on-going learning and development.
  • Full driver - s licence.
  • Strong and successful experience in leading and delivering comprehensive assessments, and working in collaboration with people and their whānau to ensure best possible health and wellbeing outcomes.
  • Record of working successfully within complex systems to collaborate and facilitate positive outcomes.
  • Experience in using critical thinking and judgement on how best to allocate resources to support the recommendations from the assessment.
  • Proficiency in using technology within the workplace.
  • A high standard of written and spoken English.
