About the role
Based in Ashburton hospital, Ashburton Community services is run from hospital and specialist services as part of Ashburton Hospital. Ashburton & Rural Health Services provides a range of Inpatient, Day patient, Outpatient and Community based health services to the community.
The Community Service team at Ashburton Hospital has an opportunity for a Registered Nurse/Allied Health Professional to join their team as a Needs Assessor and Service Coordinator for Older Person - s health.
This role requires a comprehensive clinical assessment of older person - s medical, rehabilitation and support needs to enable them to remain in their current environment. You will be supporting older people to maintain their independence using a restorative model at home by providing necessary support or assist them in the decision making process around placement. You will liaise with the multi-disciplinary team, coordinating referrals and empowering the client and their family to ensure a positive outcome.
This role will require you to use the electronic InterRAI Assessment tool.
InterRAI trained is preferred but not essential, as training can be provided.
Roles available;
- 0.9 fixed term until September 2026
- 0.8 fixed term until October 2026
Skills and experience
A NZ Registered or Enrolled Nurse with current Annual Practising Certificate.
