About the role:
We are seeking an experienced and passionate Primary Care Clinical Liaison (PCL) to join us at Health New Zealand- Te Whatu Ora Counties Manukau. In this pivotal role, you’ll act as a critical link between general practice teams, hospital services, community providers, and our organisation - working to improve communication, patient outcomes, and equity across the system.
As the PCL, you’ll provide expert clinical input to a range of improvement projects, support the development and implementation of integrated care models, and help build system-wide connections between primary and secondary care services.
This role is ideal for a current General Practitioner (GP) or Nurse Practitioner (NP) with a strong presence in the Counties Manukau district and a passion for system improvement and collaboration.
Key Responsibilities:
- Provide clinical leadership and primary care insight into service development, clinical pathways, and quality improvement initiatives.
- Support communication between hospital services and primary care providers.
- Facilitate the integration of care across primary, community, and hospital sectors.
- Lead or contribute to clinical guideline and pathway development.
- Work closely with Māori and Pacific providers to improve equity and health outcomes.
- Support training, CME, and development for general practices.
- Assist with complaint resolution and system learning following adverse events.
About You:
- NZ Registered General Practitioner (RNZCGP or RNCUC) or Nurse Practitioner.
- Recent experience practising in the Northern Region.
- 4+ years experience in primary care in NZ.
- Proven relationship-building, leadership, and facilitation skills.
- Deep understanding of primary care systems and challenges.
- Committed to advancing Te Tiriti o Waitangi and improving equity for Māori and Pacific communities.
