About the Role

As an inpatient social worker you will:

  • Provide psychosocial assessments, interventions and discharge planning in collaboration with the wider multidisciplinary team.
  • Identify psychosocial risk factors such as family harm, elder abuse and child protection. The social worker will formulate safety plans in collaboration with patients, whanau and the clinical team to ensure safety during and after discharge.
  • Support patients and whanau to navigate complex health journeys and discharge planning.
  • Advocating for patient’s rights, equity and access to resources.
  • Collaborating with multidisciplinary teams to ensure safe and timely discharges.
  • Actively participate in team meetings and contribute to care planning and discharge co-ordination.
  • Engage in quality improvement initiatives and professional development activities.

About you

  • Holds a New Zealand recognised qualification in Social Work (BSW, BASS, MSW, MA applied)
  • Current NZ Social Work Registration with SWRB
  • You will be required to maintain your current competency based practising certificate
  • Holds ANZASW Membership or other relevant professional body
