About the Role

Clinically support mental health service users / tangata whai ora during periods when they are experiencing moderate to severe periods of psychiatric illness that require assessment, treatment and support to assist the individual and their whānau and supports along the recovery journey, back to primary health care. Our client group is diverse, and we cover a broad-spectrum both socioeconomically and culturally. Specialist services also cater to specific population groups e.g. Māori Mental Health, Health Pacifica, Maternal or Community Drug and Alcohol Services.

The Social Worker is responsible for providing clinically effective tangata whai ora assessment, treatment, intervention and goal setting with tangata whai ora. This includes treatment / recovery plans developed together with tangata whai ora and family / whānau. The role also involves on - going monitoring and discharge panning.

Along with case management the Social Worker also provides individual and group work, education and providing sound clinical and cultural knowledge in the MDT discussions. The Social Worker will work closely with tangata whai ora / whānau and community agencies to achieve the outcomes agreed by the multidisciplinary process. The Social Worker will also contribute to their own professional development activities as well as wider social work activities within the district.

About the Team/Service Location

We work from a multi-disciplinary team (MDT) framework and the social worker is an important part of the MDT. We work closely with tangata whai ora and whānau to meet individual needs that support recovery and progress through a rehabilitation pathway that has a focus on the development of personal skills enabling people to reintegrate into local communities and eventual discharge. The Social Worker works closely with the nurse keyworker for each individual to ensure actions are completed in a timely manner - communication is an important part of each MDT members skill set.

About you

Experienced Social Worker with understanding of mental health, recovery and strengths-based models. You need excellent communication skills working with tangata whai ora, whanau and the wider MDT as well as external stakeholders. Your ability to plan and prioritize your case load is essential in this busy clinical setting.

Essential qualifications, skills and experience

  • Previous experience in working as a social worker with mental health and disability tangata whai ora in a public sector health setting would be desirable.
  • Registered Social Worker with current annual practicing certificate;
  • Current full NZ driver - s licence with ability to drive a manual and automatic car;
  • Proficiency in Microsoft Office, Word, Outlook, PowerPoint, Internet resources and e-mail;
  • A high standard of written and spoken English.

  • A commitment and understanding of the Te Tiriti o Waitangi (and application to health) and a willingness to work positively in improving outcomes for Māori.
  • Working collaboratively with other clinicians.
  • Ensuring that they follow through on their work.
  • Delivering identified outcomes.
