About the Role

We are seeking a dedicated Clinical Needs Assessor to provide screening, triaging, and coordination of appropriate services based on comprehensive assessments. In this role, you will identify the need for specialist assessments, therapeutic interventions, and ongoing support for individuals aged 65 and over, as well as those aged 16-64 with age-related disabilities or long-term chronic health conditions.

Assessments will be conducted both in the home and via phone. This role will require occasional travel throughout Northland to complete home assessments.

About the Team/Service/Location

The NASC team provides Needs Assessment & Service Coordination for older people and people with chronic health conditions with high needs who have a disability requiring long term supports, and for people of any age who require acute short-term community home support.

About you

We’re seeking a dedicated individual with a commitment to supporting people with age-related disabilities and complex health needs. Strong verbal and written communication skills are essential to thrive in this role.

  • Tertiary qualification in either Nursing, Occupational Therapy, Social Work or Physiotherapy with a current practising certificate
  • Recent clinical experience with proven assessment skills
  • Current, clean full NZ drivers licence
  • Advanced computer skills, including Microsoft Excel, Word, and Outlook
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth

Previous experience in working in the community and a working knowledge of services for older people and the disability sector is desirable, however not essential to apply. If you’re ready to bring enthusiasm and expertise to a meaningful position, we’d love to hear from you!
