Mo te tunga | About the role

Complex care encompasses those situations where the non-clinical demands of case management exceed the resources of the primary care team to provide. This includes both physical and psychological illness and injury.

Working within a multidisciplinary environment, this key, non-clinical role will have regional responsibility for supporting patients and health professionals through care planning, coordination and service navigation, maintaining effective communication between all stakeholders involved in the care of the service person.

Along with at least 5 years' experience in a customer based supported environment you will demonstrate the following:

  • Excellent communication and relationship management skills.
  • A high level of self- motivation, able to work autonomously and manage any issues as they arise.
  • Well organised and experienced in coordinating people and resources effectively.
  • Flexible and adaptable to the changing needs and requirements of clients.
  • Pro-actively able to manage networks and act as a link between relevant internal and external health services to support the requirements of an individualised plan.
  • Ability to support the clinical team in identifying any gaps in the continuity of care for an individual.

Regular travel between all sites within the Auckland region (Papakura, Devonport, Whenuapai) will be part of this role.

The successful applicant will have the option to work from Whenuapai or Devonport.

Higher Level Security Clearance

  • You must be a New Zealand citizen or you must hold New Zealand Permanent Residence or have a current New Zealand Residency Class Visa.
  • At minimum, you must hold New Zealand Permanent Residence or have a current New Zealand Residency Class Visa, and you must be a citizen of and have lived in Australia, Canada, United Kingdom, United States of America or New Zealand continuously for the last ten (10) years.
  • You must have a ten (10) year background history that can be verified and assessed appropriately by the NZ Security Intelligence Service (NZSIS) to determine if you are suitable for a security clearance at this level.
